By purchasing a ticket from fstbal.com, I, the undersigned participant ("Participant"), acknowledge and agree to the following terms and conditions for my participation in activities at FSTBAL.

1. Acknowledgment of Risk

I am aware that the activities I am voluntarily participating in at FSTBAL are inherently hazardous. I understand that my participation could result in serious injury, including permanent disability, or even death. I knowingly and voluntarily participate in these activities with full knowledge of the risks involved, and I agree to assume any and all risks of injury, death, or property damage, whether those risks are known or unknown to me.

2. Release and Waiver of Liability

In consideration for being permitted by FSTBAL, the State of New York, the County of New York, and any lessor of the FSTBAL premises ("Lessor") to participate in these activities and use the FSTBAL premises and facilities, I hereby forever release and discharge FSTBAL, the State of New York, the County of New York, the Lessor, any FSTBAL affiliated organization, and their respective directors, officers, coaches, employees, and agents (collectively, "Releasees") from any and all actions, claims, or demands that I, my assignees, heirs, guardians, next of kin, spouse, and legal representatives now have or may have in the future for injury, death, or property damage. This release includes, but is not limited to, claims related to:

(i) My participation in these activities. (ii) The negligence or other acts, whether directly connected to these activities or not, and however caused, by any Releasee. (iii) The condition of the premises where these activities occur.

I also agree that I will not make a claim against, sue, or attach the property of any Releasee in connection with any of the matters covered by this release.

3. Acknowledgment of Understanding

I have carefully read this agreement and fully understand its contents. I am aware that this is a legally binding release of liability and a contract between myself and FSTBAL, the State of New York, the County of New York, and the Lessor. I am checking this box and agreeing to these terms of my own free will.

I verify that the dangers of the activities and the significance of this Release and Waiver were explained to me, and that I fully understood them.
